Payment: As a self-pay provider, payment is due at the time of service. Cash, checks, and all major credit cards are accepted as payment.
Cancellation Policy: Failure to arrive for your scheduled appointment without notifying Dr. Schwartz at least 24 hours in advance will result in being charged for your full session. Appointments scheduled on Monday must be canceled on the previous Thursday during office hours. Thank you!
